Welcome to Valley Adult Day Health Care Center

We are pleased to inform you that we are fully operating providing face to face  services. Valley Adult Day Health Care Center is a community-based program providing a variety of Medical, Therapeutic and services for adults. Our objective is to assist with the activities of daily living and to prevent premature institutionalization by offering stimulating and enjoyable group experiences.

Our Services

Our services include, but are not limited to the following:

Multi-Disciplinary Team Assessments

The multidisciplinary team in collaboration with the participant and/or the participant’s caregiver assess each participant’s need for services to create an individualized plan of care.

Medical & Nursing Services

The nursing and medical team conducts an evaluation on each participant  to determine the needs for personal care and assist in activities of daily living such as bathing, grooming, toileting and eating.

Medical Social Services

The social services team provide counseling and referrals to available community resources. They promote peer group relationships through problem-centered discussion groups. The social work staff serve as a liaison with the participant’s family/caregiver and home.

Nutritional Counseling

Therapeutic diets are prepared and served as ordered by the registered dietician. Nutritional services also includes individual dietary counseling and educational classes.

Physical, Occupational, and Speech Therapy

The team provides treatment to relieve participant’s pain, develop and restore their overall functioning.

Psychological Services

The psychological team provides services in a therapeutic setting to help restore dignity and maintain good mental health to all participants.

Activity Program

The activity program at Valley Adult Day Health Care focuses on providing activities that meet  the needs and interest of each participant. The main focus is to encourage participants towards the restoration of self-care, and prevent further mental or physical deterioration.

Transportation

Medical transportation is provided to and from the participant’s home.
