Jewel assesses the nursing needs of the participant as a member of the multidisciplinary assessment team. She develops an individualized plan of care for each participant including short term and long-term goals. She is responsible for the assessment of the participant's personal care needs and assistance with activities of daily living; supervises the provision of these services. She also provides supportive and restorative nursing services as defined in plan of care. She monitors, administers, and records prescribed medications according to regulations. Observes and monitors the health status of all participants. She serves as liaison to participant’s personal physicians; keeps personal physicians informed of any marked changes in the participant's conditions. Jewel also provides health education and counseling to the participant and family as indicated in the plan of care.
